Program Type: Skilled Nursing Facility

To Make Referrals: Residents are referred through contracted counties in California. We accept referrals Monday - Friday, 8:00 am to 5:00 pm. Please call our main number at 510-261-9191.

Who We Serve: Adult residents of contracted counties throughout California who require 24-hour medical, nursing and rehabilitative services. Our residents meet the following criteria:

  • Our residents meet the following criteria:

    • They are conserved

    • They have a primary neurological diagnosis

    • They have Medi-Cal coverage

    • Coverage of Contracted Patch Rate

    • Ages 18 to 64

Office Hours: Monday - Friday: 8:00 am. - 5:00 pm. On-call staff at the program are available 24 hours a day, 7 days a week

About the Program: At Garfield Neurobehavioral Center, we have a blend of medical and psychiatric expertise to strengthen our focus on whole person care. Our team includes a psychiatric/adult nurse practitioner, social worker, behavioral specialist, Director of Nursing, and physician who work together to address our residents’ medical and psychological issues. Every staff member is trained in behavioral de-escalation practices from the Crisis Prevention Institute (CPI) to maintain a safe environment.

Garfield Neurobehavioral Center meets a unique need for our contracted counties. We accept conserved individuals who have difficult-to-manage behavior disorders for whom lower levels of care are not appropriate. We can fill the expertise gap between psychiatric and medical settings.
